Work Schedule: Monday - Friday, with shifts from 8am-6:30pm & 9:30am-8pm, including rotating weekends once a month.

Sign-On Bonus: A $5,000 sign-on bonus is available after 90 days of employment.

Key Responsibilities:

  • Assess physician medication orders for appropriateness regarding drug selection, dosage, potential interactions, and administration routes. Provide recommendations for therapy adjustments to physicians and facility staff as necessary.
  • Maintain a comprehensive patient database, including relevant medical history, clinical data, and current medications.
  • Ensure that necessary laboratory tests for monitoring high-risk conditions are ordered, evaluated, and discussed with the healthcare team.
  • Collaborate with the healthcare team to offer clinical and technical support as required.
  • Document all communications with healthcare professionals and patients in the medical record.
  • Implement policies and procedures for the safe handling, storage, distribution, and disposal of medications.
  • Maintain compliance with all required documentation as per federal and state regulations.
  • Report any medication-related errors to the Pharmacy Manager.
  • Assist facility staff with procedures related to the safe procurement and administration of medications.
  • Perform additional tasks as assigned.

Qualifications:

  • Bachelor's Degree in Pharmacy; Pharm D preferred.
  • Minimum of one year of experience in long-term care.
  • Licenses/Certifications as required by regulations and business needs.
  • Detail-oriented, goal-driven, and a team player with strong oral communication skills.
