Job Summary:

A-Line Staffing Solutions is seeking a skilled Long Term Care Pharmacist to join our team. As a Long Term Care Pharmacist, you will be responsible for maintaining patient/resident databases, ensuring laboratory tests are ordered and evaluated, and collaborating with the healthcare team to provide clinical and technical support.

Key Responsibilities:

  • Database Management: Maintain accurate and up-to-date patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medication.
  • Lab Test Management: Ensure laboratory tests are ordered, evaluated, and discussed with the healthcare team as needed.
  • Collaboration: Work with the healthcare team to provide clinical and technical support, including documentation of communication with physicians, nurses, and other team members.
  • Policy Implementation: Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals, as well as disposal of drugs.
  • Error Reporting: Inform the Pharmacy Manager of medication-related errors.
  • Training and Support: Assist facility/agency staff with procedures related to safe and effective methods of procuring, maintaining, administering, and disposing of drugs.
  • Additional Tasks: Perform other tasks as assigned by the Pharmacy Manager.

Requirements:

To be successful as a Long Term Care Pharmacist, you will need to have a strong understanding of clinical and technical aspects of pharmacy practice, as well as excellent communication and collaboration skills.
