A-Line Staffing Solutions is seeking a highly skilled Long Term Care Pharmacist to join our team. As a key member of our healthcare team, you will be responsible for providing clinical and technical support to patients and residents in long term care settings.
Key Responsibilities- Maintain accurate and up-to-date patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medication information.
- Ensure that laboratory tests for monitoring high-risk conditions are ordered, evaluated, and discussed with the healthcare team.
- Collaborate with the healthcare team to provide clinical and technical support as needed.
- Document all communication with physicians, nurses, and other healthcare team members in the medical record.
- Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals.
- Obtain and maintain required documentation in accordance with federal and state regulatory agencies and policy.
- Inform the Pharmacy Manager of medication-related errors.
- Assist facility staff with procedures related to safe and effective methods of procuring, maintaining, administering, and disposing of drugs.
- Bachelor's Degree required; PharmD preferred.
- 1+ year of pharmacist experience in long term care settings.
- Mandatory attendance for the first 90 days.
- Licensed as a pharmacist in the state of VA.
