Overview
The Clinic Registered Nurse (RN) is responsible for delivering high-quality nursing care in alignment with InnovAge policies and relevant regulations to participants.
Key Responsibilities
The RN will:
- Assist in the assessment, testing, and treatment of participants.
- Execute orders from primary care providers.
- Prioritize and manage participant and staff phone inquiries effectively.
- Coordinate external appointments for participants as needed.
- Administer prescribed medications and treatments, providing guidance to participants on self-administration and monitoring adherence.
- Maintain accurate documentation and participant medical records.
- Offer health education and counseling to participants and their families.
- Monitor, document, and communicate participant conditions and responses to treatments to healthcare providers.
- Support clinical education and supervision of CNA Day Center staff.
- Develop, implement, and maintain individualized nursing care plans.
- Oversee LPN/LVN staff and Medical Assistants, evaluating clinic effectiveness in the absence of the Nursing Services Director/Clinic Nurse Supervisor.
- Ensure exam rooms and triage areas are adequately stocked with necessary supplies.
- Engage actively in interdisciplinary team meetings, family discussions, and clinic gatherings.
- Contribute to the interdisciplinary team in creating Plans of Care for participants.
- Be available for after-hours and holiday coverage as required.
- Adhere to all regulatory and policy guidelines.
Qualifications
Required:
- Valid Registered Nurse License issued by the State
- Current Basic Life Support (BLS) certification or ability to obtain it
- Minimum of 1 year experience working with elderly or frail populations
Preferred:
- Bachelor's degree in Nursing
- At least 1 year of experience in acute care nursing
- 3+ years of healthcare experience with a focus on geriatrics
- Proficiency in Spanish, Russian, Punjabi, or Hmong
