This division serves as a virtual support system for Medicaid recipients facing chronic health challenges. Our model emphasizes comprehensive care management, ensuring that all caregivers collaborate effectively to meet the diverse needs of our patients. Our team of care coordinators plays a vital role in connecting patients with essential social services, including housing assistance, transportation for medical appointments, medication reminders, educational resources, and financial aid.

Key Responsibilities:

  1. Conduct thorough research and verification of patient enrollments in compliance with regulatory standards.
  2. Engage with outreach specialists to gather any missing documentation before submission to the electronic portal.
  3. Provide guidance to the outreach team regarding the application process and necessary updates.
  4. Document communication notes and outcomes related to enrollment with health providers.
  5. Ensure compliance by verifying that all data sources through eCares and Sharepoint are complete and accurate.
  6. Review and confirm that all documents are ready for upload into the health home systems.
  7. Address inquiries regarding enrollment status and submission updates from outreach teams.
  8. Support the administrative objectives of the department.
  9. Adhere to best practices and clinical standards, complying with departmental and state regulations.
  10. Perform additional duties as required.

Qualifications:

  • A high school diploma or equivalent is mandatory.
  • An associate's degree is preferred; relevant experience may be considered in lieu of a degree.
  • Exceptional organizational, communication, and interpersonal skills are essential.
  • Experience in customer service, particularly in a provider services call center or billing environment, is highly valued.
  • Demonstrated ability to thrive in a fast-paced, deadline-driven setting.
  • Proficiency in Microsoft Office Suite and various web-based platforms is required.
  • Capability to work collaboratively as part of a team as well as independently.
  • Ability to manage multiple tasks simultaneously with efficiency.
  • Experience in a healthcare setting is advantageous.
  • Strong understanding of provider billing practices and claims processing is necessary.
  • Must be able to maintain a stationary position for extended periods.

Compensation:
$21.00-$23.00 per hour
