We are seeking a skilled Homelessness Prevention Coordinator to join our team. In this role, you will be responsible for providing professional intervention to clients at risk of homelessness and homeless clients through comprehensive support services.

About the Job:

The ideal candidate will have a Bachelor's degree in Human Services or a related field and one year of experience in a social service setting or providing direct services to individuals or families. Bilingual skills (Spanish) are an asset, and the ability to work flexible hours, including weekends and evenings, according to program needs.

Responsibilities:

  • Assess client needs and develop individualized plans to address those needs.
  • Conduct regular home visits and follow-up with clients as needed.
  • Maintain accurate and up-to-date case records, including progress notes.
  • Provide referrals to community resources and services as necessary.
  • Foster partnerships with community agencies to ensure a seamless delivery of services.

Benefits:

This full-time position offers a competitive salary range of $45,000 - $55,000 per annum, depending on experience.
