LPN Care Coordinator

We are seeking a skilled LPN Care Coordinator to join our team at Ochsner Health System. The successful candidate will assist in data collection and implementation of healthcare services, while maintaining a positive work relationship with the healthcare team.
Key Responsibilities- Assist in data collection and implementation of healthcare services
- Maintain a positive work relationship with the healthcare team
- Apply the Nursing Process to provide quality patient care
- Oversee the disease registry database and coordinate care across the continuum
- Participate in quality improvement activities and remain knowledgeable on current federal, state, and local laws and regulations
- Graduate of an accredited school of practical nursing
- 2 years of ambulatory care experience
- Preferred: 3 years of ambulatory care and home health care experience
- Proficiency in using computers, software, and web-based applications
