Job Title: CAT Case Manager

At LifeView Group, we are seeking a highly skilled and compassionate CAT Case Manager to join our team. As a key member of our behavioral health services, you will play a vital role in providing comprehensive support to individuals and families in need.

Job Summary:

The CAT Case Manager will be responsible for coordinating and delivering case management services to clients with complex needs. This includes assessing client needs, developing and implementing individualized plans, and connecting clients with community resources and services.

Key Responsibilities:
  • Conduct comprehensive assessments of client needs and develop individualized plans to address those needs.
  • Coordinate and deliver case management services, including counseling, advocacy, and support.
  • Connect clients with community resources and services, such as housing, employment, and education.
  • Collaborate with other healthcare professionals and community agencies to ensure comprehensive care.
  • Monitor client progress and adjust plans as needed.
Requirements:
  • Bachelor's degree in a related field, such as counseling, social work, or psychology.
  • Minimum of one year of experience working with children with serious emotional disturbances.
  • Ability to complete approved mental health targeted case management training within three months of hire.
  • Knowledge of available resources in the service area for children with serious emotional disturbances.
  • Ability to work independently and as part of a team.
