RN Case Manager

Job Summary:
We are seeking a skilled and compassionate RN Case Manager to join our team at Interim HealthCare. As a RN Case Manager, you will be responsible for providing high-quality patient care and services to clients in their homes. You will work closely with our interdisciplinary healthcare team to develop and implement individualized care plans that meet the unique needs of each patient.
Key Responsibilities:
- Conduct thorough assessments and evaluations of patients to identify their needs and develop effective care plans.
- Collaborate with healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
- Provide education and support to patients and their families to promote health and wellness.
- Monitor patient progress and make adjustments to care plans as needed.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements:
- Graduate of an accredited school of nursing with a current license as a Registered Nurse in the state(s) in which you practice.
- Two (2) years of current nursing experience, preferably in home health or case management.
- One (1) year of management experience preferred.
Preferred Qualifications:
- Community health experience and a wide range of Medical-Surgical experience.
Working Conditions:
This position requires occasional travel within the geographic area serviced by our office. You will be working in a typical interior/office work environment but may require frequent visits to patient/client homes. You will be exposed to communicable diseases, bloodborne pathogens, and other potentially infectious or hazardous materials, and will be required to follow extensive safety precautions.
