Job Title: RN Case Manager

We are seeking a skilled and compassionate RN Case Manager to join our team at Interim HealthCare. As a RN Case Manager, you will play a vital role in ensuring the highest quality patient care and services are provided to our in-home clients.

Key Responsibilities:
  • Perform comprehensive assessments and procedures to identify patient needs and develop individualized care plans.
  • Develop and maintain effective working relationships with discharge planners, social service workers, and other healthcare professionals.
  • Assess and review the matching of employee skills to client needs, ensuring the best possible outcomes.
  • Visit clients in their homes to evaluate the quality of care being provided and review clinical notes.
  • Complete timely and accurate clinical notes, including addressing patient progress and any concerns.
  • Effectively communicate with the interdisciplinary healthcare team to promote coordination of patient care and planning for discharge.
  • Supervise paraprofessional and licensed professional team members, ensuring seamless care delivery.
  • Secure equipment and supplies, and coordinate placement for use in the home.
  • Refer clients to other services as needed, ensuring comprehensive care.
Requirements:
  • Graduate of an accredited school of nursing with a current license as a Registered Nurse in the state(s) in which you practice.
  • Two (2) years of current nursing experience, with a preference for community health and medical-surgical experience.
  • One (1) year of management experience, with a preference for supervisory roles.
  • Able to perform and evidence of current CPR certification for adults, children, and infants.
  • Able to demonstrate basic understanding of computers and workplace software.
  • Able to work with people, function well under stress, use clinical judgment, be creative, and accept challenges.
  • Able to effectively communicate in English with all levels of the workforce, both clinical and non-clinical.
  • Able to perform and prioritize multiple functions or tasks.
  • Able to read and interpret technical instructions related to patient care.
  • Able to visually and aurally observe and assess patients.
  • Able to effectively deal with multiple changes based on patient needs and scheduling.
Working Conditions:
  • Able to occasionally travel within the geographic area serviced by our office.
  • Meets the applicable health requirements to provide patient care per applicable law or regulation.
  • Work is normally performed in a typical interior/office work environment but may require frequent visits to patient/client homes.
  • Occasional exposure to communicable diseases, bloodborne pathogens, and/or other potentially infectious or hazardous materials and situations that require following extensive safety precautions and may include the use of protective equipment.
