Requirements:

  • Graduate of an accredited school of nursing with a current RN license in the state(s) of practice.
  • Minimum of 2 years of current nursing experience, with a focus on home health or case management.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work independently and as part of a team, with a focus on patient-centered care.

Preferred Qualifications:

  • Experience working in community health or home health settings.
  • Knowledge of case management principles and practices.
  • Ability to work with electronic medical records and other healthcare software.

Working Conditions:

This position requires occasional travel within the geographic area serviced by our office. You will be working in a typical interior/office work environment, with frequent visits to patient/client homes. You will be exposed to communicable diseases, bloodborne pathogens, and other potentially infectious or hazardous materials, and will be required to follow extensive safety precautions.
