About the Role

We are seeking experienced Home Health Registered Nurses to join our team. As a Home Health RN, you will have the opportunity to provide one-on-one care to patients in the comfort of their own homes. You will work closely with our patients to develop personalized care plans, conduct home visits, and provide skilled interventions as ordered.

Responsibilities
  • Conduct comprehensive assessments and develop care plans in collaboration with Case Managers
  • Provide skilled interventions, including teaching and other therapeutic actions
  • Coordinate patient discharges and participate in the planning process
  • Work closely with patients and families to clarify goals and incorporate them into care plans
Requirements
  • Current Registered Nurse (RN) license in the state of Arizona, in good standing
  • CPR Certification (demonstration course required)
  • Valid Arizona Fingerprint Card
  • One (1) year of home health experience preferred but not required
