Home Health Registered Nurse Opportunity

About the Role

As a Home Health Registered Nurse, you will be responsible for providing high-quality patient care in a home setting. You will work closely with patients, families, and healthcare teams to develop and implement personalized care plans that meet the unique needs of each individual.

Key Responsibilities
  • Conduct comprehensive assessments of patients' physical and emotional needs
  • Develop and implement individualized care plans that address patients' goals and priorities
  • Provide skilled nursing interventions, including wound care, medication management, and education
  • Collaborate with patients, families, and healthcare teams to ensure seamless care transitions
  • Document patient progress and outcomes in a timely and accurate manner
Requirements
  • Current Registered Nurse (RN) license in the state of Arizona, in good standing
  • CPR Certification (demonstration course required)
  • Valid Arizona Fingerprint Card
  • One (1) year of home health experience preferred but not required
