Job Summary

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a PRN/Part Time - Home Risk Assessments professional. In this role, you will conduct comprehensive health assessments, medical history reviews, and provide health education to patients in their homes.

About the Position

As a Matrix Provider, you will work collaboratively with physicians, case managers, and other healthcare professionals to deliver high-quality care. You will conduct assessments, provide diagnoses and treatment plans, and refer patients to specialists as needed. You will also work with patients to develop personalized care plans and provide ongoing support and education.

Responsibilities

  • Conduct comprehensive health assessments, including medical history reviews and physical examinations
  • Develop and implement personalized care plans for patients
  • Provide health education and support to patients and their families
  • Collaborate with healthcare professionals to deliver high-quality care
  • Document patient information and care plans in accordance with Matrix guidelines and protocols

Requirements

  • Master's degree in Nursing or Physician Assistant studies
  • Current RN and NP or PA licensure in the state of practice
  • Board certification in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant
  • Strong computer skills and familiarity with EMR software
  • Excellent verbal and written communication skills

Travel Requirements

  • Ability to travel to patients' homes and other locations as needed
  • Valid state driver's license and adequate automobile insurance coverage
