Job Summary

We are seeking a skilled Nurse Practitioner to join our team as a PRN/Part Time Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments, provide medical history and diagnosis, and develop treatment plans to improve patient outcomes.

Key Responsibilities
  • Conduct adult and geriatric assessments, medical history, diagnosis, and treatment in a variety of settings, including home visits, skilled facilities, and telehealth visits.
  • Collaborate with primary care physicians, case managers, social workers, and other healthcare professionals to provide comprehensive care.
  • Develop and implement treatment plans to improve patient outcomes and reduce healthcare costs.
  • Provide education and support to patients and their families to promote healthy behaviors and disease management.
Requirements
  • Master's degree in nursing or a related field.
  • Current RN and NP licensure in the state of practice, including prescription authority.
  • Board certification in a Matrix-approved specialty.
  • Minimum of 1 year experience as a Nurse Practitioner.
  • Strong computer skills and familiarity with electronic health records.
  • Excellent communication and interpersonal skills.
