Job Summary

We are seeking a highly motivated and compassionate Senior Care Coordinator to join our team at Villas of Holly Brook and Reflections Memory Care. As a key member of our caregiving staff, you will play a vital role in facilitating the resident care team and ensuring the highest level of care for our residents.

The ideal candidate will have a strong background in caregiving, excellent communication skills, and the ability to work effectively in a fast-paced environment.

Responsibilities:

  • Facilitate and coordinate the resident care team
  • Personal care: assist residents with Activities of Daily Living
  • Coordination: participate in caregiver interviews, make daily staff assignments, implement training, mentor caregiver staff, and act as a coach to all caregivers
  • Review point of care system and 24-hour log for compliance
  • Assist with inventory and ordering of incontinence program and supplies
  • May assist the Wellness Director with obtaining medical certification and medication lists from physicians

Requirements:

  • Must be at least 18 years old or older
  • Certified Nurse Assistant (C.N.A.) preferred
  • Competent in all caregiver tasks
  • Able to work any day and shift
  • Two years' experience in assisted living, memory care, or long-term care preferred
  • Demonstrates good judgment, problem-solving, and decision-making skills
  • Able to handle multiple priorities
  • Excellent communication and interpersonal skills
  • Maintain required training hours per year
  • Able to perform essential job functions with or without reasonable accommodations

Physical Requirements:

Medium work: able to exert up to 25 pounds frequently and 50 pounds occasionally
