">We are seeking a compassionate and dedicated Care Manager to join our team at Sunrise Senior Living. As a Care Manager, you will be responsible for providing the highest degree of quality care and services to a consistent group of residents in our assisted living and reminiscence neighborhoods.
">Your key responsibilities will include:
">- ">
- Building meaningful relationships with seniors and their families through personalized assistance with activities of daily living and individualized care plans.">
- Notifying management of changes in condition and recommending adjustments to the level of care and service as needed.">
- Assisting residents in life skills and other life-enriching activities as indicated on their individual profiles.">
- Participating in the development of Individualized Service Plans (ISPs) and monthly updates.">
- Maintaining accurate records of resident progress and communicating effectively with interdisciplinary teams.">
Required Skills and Qualifications

- A high school diploma or equivalent and relevant work experience.">
- Excellent customer service skills and a passion for serving seniors.">
- Ability to work in a fast-paced environment and prioritize multiple tasks efficiently.">
- Familiarity with OSHA regulations and safety protocols.">
- Demonstrated ability to maintain confidentiality and handle sensitive information discreetly.">
- Basic computer skills and proficiency in electronic medical records software.">
