About the Role

The MDS Coordinator RN role at American Senior Communities is a challenging and rewarding position that involves providing care and compassion to patients while promoting individualized support and care of residents by coordinating assessments, managing care plans, and overseeing assessments for case mix reimbursement.

This position requires a graduate of an accredited School of Nursing, preferably BSN, with minimum one year in nursing management in the long-term care industry. The ideal candidate will have two years professional nursing experience in long-term care, acute care, restorative care, or geriatric nursing setting, along with knowledge of the Resident Assessment Instrument, Interdisciplinary Care planning process, and Minimum Data set 2.0.

Responsibilities
  • Providing care and compassion to patients through a holistic approach.
  • Acting as a positive teammate to fellow employees to create a supportive work environment.
  • Coordinating assessments using RAI tools to ensure accurate and timely information.
  • Managing the interdisciplinary care plan process to promote resident-centered care.
  • Overseeing management of resident assessments for case mix reimbursement and transmitting MDS reports in a timely manner.
  • Serving as a member of Quality Improvement committees to enhance positive resident outcomes.
Requirements
  • Demonstrates C.A.R.E. values to residents, family members, customers, and staff through exceptional communication skills.
  • Graduate of an accredited School of Nursing, preferably BSN.
  • Minimum of one year in nursing management in the long-term care industry.
  • Two years professional nursing experience in long-term care, acute care, restorative care, or geriatric nursing setting.
  • Knowledge of OBRA regulations and familiarity of current state reimbursement guidelines.
  • Knowledge of current laws and regulations that influence provision of care and services in nursing facilities.
