MDS Nurse Coordinator (Registered Nurse/RN)

Position Overview


The RN MDS Coordinator is responsible for overseeing and facilitating the accurate and timely completion and submission of interdisciplinary MDS Assessments, CAAs, and Care Plans in compliance with CMS RAI Manual Regulations and all relevant laws and standards.

Qualifications and Licensure Requirements

  • An associate's or bachelor's degree in nursing from an accredited institution
  • Active and unrestricted Registered Nurse (RN) license in the applicable state
  • Minimum of two (2) years of nursing experience, with a preference for geriatric nursing
  • CRN C Certification (clinical compliance)
  • CPR certification required upon hire or to be obtained during orientation, with ongoing maintenance throughout employment
Key Responsibilities

  • In-depth knowledge of nursing practices and regulations
  • Ability to make independent decisions when necessary
  • Familiarity with laws, regulations, and guidelines relevant to post-acute care facilities
  • Implement and interpret departmental programs, objectives, policies, and procedures
  • Exhibit proficiency in all competency areas, including patient rights and safety
  • Maintain professional relationships with colleagues and vendors
  • Uphold confidentiality of proprietary and sensitive information
  • Adhere to company policies, including those related to harassment and compliance
  • Demonstrate integrity and professionalism by following Life Care's Code of Conduct and completing required compliance training
Essential Duties

  • Coordinate and assist with the completion and submission of interdisciplinary, accurate, and timely MDS Assessments, CCAs, and Care Plans in accordance with CMS RAI Manual Regulations
  • Notify the Director of Nursing (DON) of any changes in a patient's condition identified through the MDS Assessment
  • Educate direct care staff on updates or changes to CMS RAI Manual or Skilled Nursing Facility Regulations affecting documentation
  • Participate in the review of the Interdisciplinary Comprehensive Care Plan
  • Validate that all assessments are completed and accepted into the CMS QIES system prior to billing, and inform the Business Office of any assessments not ready for billing
  • Analyze CMS Reports to identify late submissions and develop strategies to prevent recurrence
  • Attend and contribute to daily and monthly meetings as requested
  • Perform nursing duties as needed
  • Provide excellent customer service and maintain a positive attitude towards residents
  • Assist in patient evacuations when necessary
  • Exhibit regular and dependable attendance
  • Utilize reasoning skills and sound judgment in decision-making
  • Collaborate effectively within an interdisciplinary team
  • Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, walking, and reaching during work hours
  • Communicate effectively in English, both verbally and in writing
