Hospice Care Coordinator

The Hospice Nurse, RN is a vital member of our team at 3HC Home Health and Hospice. This role is responsible for coordinating all Hospice services in assigned counties, upholding the Hospice philosophy, and aligning with Agency goals, objectives, and policies.
Key Responsibilities:
1. Deliver exceptional experiences for internal and external customers, showcasing courtesy, tact, and knowledge of services provided by the agency in all interactions.
2. Provide comfort and support to patients and their families, ensuring quality of life and dignity at the end of life.
3. Maintain accurate and up-to-date electronic documentation, adhering to Hospice guidelines and COPs, and ensuring timely completion of assessments in McKesson.
4. Collaborate with physicians and the Hospice Medical Director to ensure comprehensive patient care.
5. Foster open communication with supervisors, addressing patient concerns in a timely manner, and coordinating care with the IDG team and Director of Clinical Services/Assistant Clinical Director.
6. Ensure accurate billing for services provided.
Requirements:
Licensed as a Registered Nurse, in good standing in North Carolina preferred.
Prefer Home Health/Hospice experience.
Minimum of one year experience in an Acute Care Setting.
Current CPR certification.
