Nurse Assessment Coordinator

At 1st Employment, we are seeking a highly skilled and experienced Nurse Assessment Coordinator to join our team. As a key member of our healthcare team, you will be responsible for overseeing and coordinating the federally mandated resident assessment instrument (RAI) process.

Key Responsibilities:
  • Coordinate the RAI process, including the Minimum Data Set (MDS), care area assessment, and care plan development.
  • Work with the interdisciplinary team to complete MDS assessments, analyze care areas, and develop and revise comprehensive care plans.
  • Ensure compliance with state and federal guidelines, including the Patient-Driven Payment Model (PDPM) and Medicare requirements.
  • Provide oversight of assessment completion and transmission to the national repository, reviewing final validation reports and correcting or modifying assessments as needed.
  • Coordinate care with case managers for residents utilizing managed care, health maintenance organizations (HMOs), commercial insurance, and other alternate payment models.
Requirements:
  • An active unencumbered nursing license in the state of Arkansas.
  • A registered nurse (RN) is preferred.
  • MDS certification with a minimum of 2 or more years of experience.
  • Long term care experience with a minimum of 5 or more years of experience.
