Job Title: Home Health Speech Language Pathologist

Alternate Solutions Health Network LLC is seeking a skilled Home Health Speech Language Pathologist to join our team in Burke, Virginia.

Job Description:

We are looking for a compassionate and experienced Speech Language Pathologist to provide high-quality care to our patients in the comfort of their own homes. As a Home Health Speech Language Pathologist, you will work closely with our interdisciplinary team to develop and implement individualized plans of care that address the unique needs of each patient.

Responsibilities:
  • Develop and implement individualized plans of care for patients with speech and language disorders
  • Provide home-based skilled speech therapy care to patients as directed by the attending physician
  • Educate patients and their family members on therapeutic activities and self-care techniques to improve functioning
  • Observe, record, and report patient reactions, changes, and progress to attending physician and any appropriate agency staff
  • Use activities and home exercise programs to improve swallowing, breathing, cognition, speech, and communication, and/or any other speech therapy-related function
  • Help decrease re-hospitalizations by teaching patients on preventative measures and making good decisions
  • Counsel patient and family on the disease/injury and how to manage in all phases of speech therapy
Requirements:
  • Speech Therapist with a current license in the state of employment
  • Minimum one-year experience as a ST in an acute care setting
  • Home care experience preferred
  • Valid driver's license and auto insurance in your name as a driver
  • Capable of all physical demands
