Schedule: Full-time, salaried role, Monday-Friday, regular business hours

We are seeking a skilled Nurse Care Manager to join our team. As a member of the Behavioral Health Home (BHH) Adult Service Team and the BHH Children's Service Team, you will provide nursing consultation to care coordinators, oversight of health outcomes, and collaboration with primary care partners.

About the Job:

The successful candidate will have a Registered Nurse license in good standing in the State of Maine and minimum 2 years of experience in a healthcare setting. Knowledge of recovery-based, harm reduction, trauma-informed, and strengths-based approaches is highly desirable, as is experience with electronic data collection and reporting.

Key Responsibilities:
  • Provide high-quality nursing consultation to care coordinators and peer/parent navigators.
  • Oversight of health outcomes for BHH clients.
  • Collaboration with primary care partners to ensure seamless care coordination.
