We are seeking a highly motivated and compassionate Licensed Practical Nurse to join our team as a Home Health Nurse. In this role, you will provide personalized care to patients in their homes, working collaboratively with our interdisciplinary team to achieve optimal patient outcomes.
About the Role
The ideal candidate will have a strong foundation in nursing principles and practices, with excellent communication and interpersonal skills. You will be responsible for assessing patients' needs, developing and implementing individualized care plans, and providing education and support to patients and their families.
Key Responsibilities
- Provide direct patient care, including assessments, medication management, and wound care
- Develop and implement individualized care plans, in collaboration with the interdisciplinary team
- Provide education and support to patients and their families, promoting healthy behaviors and self-care
- Communicate effectively with healthcare providers, patients, and families, ensuring seamless transitions of care
- Maintain accurate and timely documentation, adhering to regulatory requirements and organizational standards
Requirements
- Licensed Practical Nurse (LPN) licensure in the state of Maine
- High School Diploma or equivalent required; Associate's degree or higher preferred
- Basic Life Support (BLS) certification required
- Valid driver's license with reliable transportation and agency-required auto liability insurance
