About the Role

As a Nurse Case Manager, you will play a critical role in coordinating care, developing treatment plans, and providing support to patients and their families.

Key Responsibilities
  • Conduct thorough assessments of patients' medical and psychosocial needs
  • Develop and implement individualized treatment plans that address specific patient goals and outcomes
  • Collaborate with interdisciplinary teams to ensure comprehensive care coordination
  • Provide education and support to patients and their families on disease management and self-care
Requirements

To be successful in this role, you must have:

  • A current RN license in the state of Pennsylvania
  • At least 1 year of experience in case management or a related field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
