Position Overview:
The Cape & Islands Adult Community Clinical Services (ACCS), under the Department of Mental Health (DMH), is in search of a motivated individual to deliver essential care services within a residential group home environment.
Key Responsibilities:
This role will serve as a primary supporter for assigned individuals, assisting them in achieving their rehabilitation objectives, completing necessary daily documentation, and upholding a Person-Centered, Trauma-Informed approach.
The selected individual will collaborate with a team in the residential group home setting, providing rehabilitative interventions to individuals with significant mental health challenges. This includes participating in the creation of comprehensive assessments and developing and executing the clients' Individual Action Plans.
Additional duties may involve coordinating appropriate services for individuals by liaising with other programs within the agency and the community, such as SSI, rehabilitation services, day programs, and medical services.
Medication Management:
Knowledge of all medications utilized within the program is crucial, as the individual will be responsible for administering medications and educating clients on their proper use.
Therapeutic Environment:
The incumbent will foster a therapeutic atmosphere through professional conduct and attitude, promoting quality care and growth for those served.
Transportation:
Transporting clients to appointments or activities using a program vehicle or personal vehicle is a necessary aspect of this role.
Shift Information:
This position requires a commitment of 40 hours per week, with specific shifts and days off to be determined. Candidates are being sought for both Day and Evening shifts.
Day Shift: 7:00 am to 3:30 pm
Evening Shift: 3:00 pm to 11:30 pm
Mandatory overtime may be required.
Essential Duties:
1. Engage individuals, both individually and in groups, in developing meaningful activities, including recreational, educational, and wellness initiatives, to enhance their quality of life.
2. Assist in formulating and executing Individual Action Plans through direct observation and engagement with the individuals served.
3. Provide rehabilitative interventions, employing coping strategies and safety planning to help individuals build skills and access resources to meet their treatment goals.
Documentation:
Maintain accurate records for individuals served by documenting in electronic and paper formats, ensuring compliance with DMH regulations.
Qualifications:
Applicants should possess the ability to follow written and oral instructions, perform basic mathematics, and communicate effectively. Experience in human services, particularly with individuals facing mental health challenges, is preferred.
Special Requirements:
Employees must pass MAP certification within six months of employment and maintain this certification while working in the program. A valid class D Motor Vehicle Operator's license is also required.
