Individualized Planning Process Facilitation Specialist

About the Role

This position is a critical part of the State of New Mexico's efforts to ensure the well-being of children and families in our state. As an Individualized Planning Process Facilitation Specialist, you will play a key role in supporting families through difficult situations using community-based prevention and intervention programs.

Key Responsibilities
  • Conduct family-centered meetings (FCMs) and Individualized Planning Meetings (IPMs) to support families in achieving their goals and improving their well-being.
  • Provide consultation and training to staff on the Individualized Planning Process and other relevant topics.
  • Work collaboratively with internal and external partners to ensure the provision of quality services and achieve the goals of safety and well-being for children and families.
  • Document efforts and plans in FACTS from staffings, other facilitations, and family group decision making (FGDM) following standards and format.
Requirements

To be successful in this role, you will need:

  • A Bachelor's degree in Social Work, Guidance and Counseling, Counseling, Psychology, Sociology, or Criminology from an accredited college or university.
  • Licensure in New Mexico as a Doctoral Level Psychologist, Psychologist Associate, LISW, LMSW, LPC, LPCC, LMHC, or LMFT, or eligibility for such licensure in accordance with New Mexico requirements.
  • Two years of experience working with communities on health or social service-related matters, social work/case management experience, behavioral health, and/or healthcare.
Working Conditions

This role requires:

  • Valid driver's license.
  • Pre-employment background investigation.
  • Ability to work in an office setting with regular periods of video display terminal and keyboard usage.
  • Flexibility to work late hours, weekends, and callback work as needed.
