Position Overview

Role Summary

The Registered Nurse Case Coordinator (RNCC) plays a vital role in delivering intake and case management services to high-risk individuals within the Mosaic Medical Community Health Home. This demographic may include patients facing intricate medical challenges, as well as socioeconomic and mental health issues.

Utilizing the Nursing Process, the RNCC is tasked with assessing, planning, implementing, coordinating, monitoring, and evaluating a range of options and services aimed at enhancing the patient's overall health. The RNCC will exhibit and apply a thorough understanding of comprehensive case management principles, patient-centered approaches, and culturally sensitive care coordination for complex patient needs.

RNCCs may focus specifically on prenatal or newborn care.

Key Responsibilities

  • Oversee a designated group of high-risk patients with the objective of improving their health outcomes and reducing the need for hospital admissions and emergency services.
  • Collaborate with Quality, Value Improvement, and Population Health teams to identify patients at high risk.
  • Apply evidence-based practices and established protocols for managing chronic conditions.
  • Incorporate clinical guidelines, preventive measures, and approved protocols to create tailored, patient-centered care plans.
  • Conduct follow-ups with patients and families regarding care transitions, including medication reviews, timely follow-up appointments, patient education, and care coordination.
  • Evaluate the health, educational, and psychosocial needs of patients and their families, and develop, implement, and assess care plans accordingly.

Required Skills & Knowledge

  • Essential: Advanced nursing process capabilities, critical thinking, and problem-solving skills. Exceptional written, verbal, and interpersonal communication abilities. Strong organizational skills and familiarity with patient interactions via telephone. Basic typing and computer proficiency, particularly with Microsoft Windows.
  • Desirable: Experience with Electronic Health Records (EHR), particularly EPIC. Proficiency in Spanish is preferred. Engagement in quality improvement initiatives. Knowledge of clinical system design and development. Experience with Motivational Interviewing. Understanding of health insurance policies and regulatory standards, including CLIA and OSHA.

Qualifications

Education

Preferred:

  • Bachelor's degree or higher in Nursing.

Licenses & Certifications

  • CPR Certification
  • Valid Driver's License
  • Registered Nurse License

Experience

Required:

  • A minimum of two years of clinical nursing experience in acute or outpatient care settings, with a preference for experience in managing medically complex patients or case management in an outpatient environment.
