Position Overview: The RN Care Coordinator plays a pivotal role within the clinic care team, offering essential support to patients and managing care plans through the nursing process.

The primary objective is to deliver personalized education, coaching, and follow-up to enhance patients' self-management capabilities.

Interventions are designed to assist patients and populations in adhering to treatment protocols and sustaining their quality of life amidst chronic conditions and complex health issues.

The RN Care Coordinator not only provides direct patient care but also ensures seamless coordination among all parties involved in the patient's care, including primary care teams, medical specialists, hospitals, and health plans.

Key Responsibilities:

  • Coordinate care for complex patients utilizing evidence-based practices.
  • Manage transitional care, including follow-up after emergency room visits and hospital stays.
  • Conduct medication reconciliation for patients at moderate to high risk post-discharge.
  • Develop and oversee care plans in partnership with primary care providers and the care team.

Required Skills and Knowledge:

  • Exceptional nursing process skills.
  • Strong critical thinking and problem-solving abilities.
  • Excellent communication skills, both written and verbal.
  • Familiarity with client interactions via telephone.
  • Knowledge of community resources.
  • Basic typing and computer proficiency, particularly with Microsoft Windows.

Preferred Qualifications:

  • Experience with Electronic Health Records (EHR), particularly EPIC.
  • Fluency in Spanish is advantageous.
  • Engagement in quality improvement initiatives.
  • Understanding of health insurance plans and regulatory standards, including CLIA and OSHA.
