Position Title: Operational Staff Pharmacist II

Department: Clinical Pharmacy and Pharmaceutical Services

Job Overview:
The Operational Staff Pharmacist II plays a crucial role in the delivery of high-quality pharmaceutical care. As integral members of the healthcare team, they are responsible for ensuring effective and safe medication management for patients.

Key Responsibilities:
1. **Medication Management:** Evaluate and oversee patient medication regimens to ensure appropriateness and effectiveness.
2. **Patient Education:** Provide guidance to patients and their caregivers on the safe and effective use of medications, empowering them to manage their therapies actively.
3. **Therapeutic Monitoring:** Continuously monitor patient outcomes and adjust pharmaceutical care plans as necessary to achieve optimal therapeutic results.
4. **Collaboration:** Work closely with healthcare professionals to disseminate vital drug information and participate in patient care discussions.
5. **Quality Assurance:** Engage in the design and execution of drug utilization reviews, audits, and other related studies to enhance pharmacy services.
6. **Leadership:** May oversee pharmacy assistants and interns, and take part in teaching pharmacy students and residents.

Qualifications:
- Must possess a Doctor of Pharmacy degree (PharmD) or equivalent, accredited by the National Board of Pharmacy.
- For international graduates, a Foreign Pharmacy Graduate Examination Committee (FPGEC) Certification is required.

Preferred Qualifications:
- Completion of an ASHP-accredited PGY1 Pharmacy Residency or a minimum of three years of relevant pharmacy experience.
- Advanced practice board certification is advantageous.

Licensure:
- A current and valid pharmacy license in good standing is mandatory.
- Basic Life Support (BLS) certification from a recognized provider is required.
