Overview:
The Operational Staff Pharmacist II plays a crucial role within the health care team, contributing to the delivery of high-quality and cost-effective pharmaceutical care. This position is essential in promoting patient health through effective medication management.
Key Responsibilities:
1. Evaluate and manage patient medication regimens to ensure appropriateness and safety.
2. Educate patients and their caregivers on the proper use of medications, empowering them to take an active role in their health management.
3. Advocate for the safe and effective use of medications among healthcare professionals.
4. Monitor and assess therapeutic outcomes, ensuring optimal patient care.
5. Develop comprehensive pharmaceutical care plans tailored to individual patient needs.
6. Provide drug information and support to healthcare staff.
7. Participate in interdisciplinary patient care conferences and contribute to drug utilization reviews and audits.
8. Oversee pharmacy programs that may require specialized knowledge and skills beyond those of a Staff Pharmacist I.
9. Mentor and train pharmacy students and residents, fostering their professional development.
Qualifications:
Required:
- Doctor of Pharmacy (PharmD) degree from an accredited institution.
- Current pharmacy license in good standing in the relevant state.
- Basic Life Support (BLS) certification.
Preferred:
- Completion of an ASHP-accredited PGY1 Pharmacy Residency or equivalent experience.
- Advanced practice board certification (e.g., Board of Pharmacy Specialty certificates).
