Patient Services Coordinator

We are seeking a highly skilled Patient Services Coordinator to join our team at CenterWell Home Health. As a key member of our care coordination team, you will play a vital role in ensuring the seamless delivery of high-quality patient care.
Key Responsibilities- Schedule visits and communicate with field staff, patients, physicians, and other stakeholders to maintain proper care coordination and continuity of care.
- Assist with day-to-day office and staff management, including managing schedules, editing schedules for agents calling in sick, and updating agent unavailability in the worker console.
- Initiate infection control forms as needed and send completed reports to the HR department.
- Provide backup support during busy times, including receiving calls from field staff and assisting with weekly case conferences.
- Maintain the client hospitalization log, including entering coordination notes and sending electronic logs to relevant staff.
- Complete requested schedules as tasks appear on the action screen, ensuring staff are scheduled for skilled nurse/injection visits unless an aide supervisory visit is scheduled in conjunction with the injection visit.
- Process reassigned and rescheduled visits, and ensure supervisory visits are scheduled.
- Run scheduling reports, including the Agent Summary Report and Missed Visits Done on Paper Report.
- Prepare weekly agent schedules, performing initial reviews for productivity and geographic issues before forwarding to the Branch Director for approval.
- 1 year of home health experience required.
- Excellent communication skills and ability to interact well with diverse individuals.
- Strong organizational skills and ability to manage and prioritize multiple assignments.
- Prior packet review/QI experience preferred.
- Coding certification preferred.
