Patient Services Coordinator, Home Health Specialist

We are seeking a highly skilled and compassionate Patient Services Coordinator to join our team at CenterWell Home Health. As a key member of our care team, you will play a vital role in ensuring the delivery of high-quality, patient-centered care to our clients.
Key Responsibilities- Coordinate patient care services, including scheduling visits and communicating with field staff, patients, physicians, and other healthcare professionals.
- Manage patient schedules, ensuring timely and efficient delivery of care services.
- Assist with day-to-day office and staff management, including updating agent unavailability in the worker console.
- Initiate infection control forms as needed and send completed reports to HRD for upload in the worker console.
- Serve as backup during busy times, including receiving calls from field staff and assisting with weekly case conferences.
- Refer clinical questions to the Branch Director as necessary.
- Maintain the client hospitalization log, including entering coordination notes and sending electronic logs to all office, field, and sales staff.
- Complete requested schedules as tasks appear on the action screen.
- 1 year of home health experience.
- Prior packet review/QI experience preferred.
- Coding certification is preferred.
