Patient Services Coordinator LPN Home Health Specialist

We are seeking a highly skilled and compassionate Patient Services Coordinator LPN to join our team at CenterWell Home Health. As a key member of our care team, you will play a vital role in ensuring the seamless delivery of high-quality home health care services to our patients.
Key Responsibilities- Schedule visits and communicate with field staff, patients, physicians, and other stakeholders to maintain proper care coordination and continuity of care.
- Assist with day-to-day office and staff management, including managing schedules for all patients and editing schedules for agents calling in sick.
- Update agent unavailability in the worker console and initiate infection control forms as needed.
- Provide backup support during busy times, including receiving calls from the field staff and assisting with weekly case conferences.
- Refer clinical questions to the Branch Director as necessary and maintain the client hospitalization log, including entering coordination notes and sending electronic logs to all office, field, and sales staff.
- Complete requested schedules for all add-ons and applicable orders, including scheduling discharge visits, OASIS collection visits, and rescheduling declined or missed visits.
- Run all scheduling reports, including the Agent Summary Report and Missed Visits Done on Paper Report, and prepare weekly Agent Schedules.
- Verify visit paper notes in the scheduling console as needed and assist with internal transfers of patients between branch offices.
- Initiate Employee/Patient Infection Reports as necessary and use your skills to make a positive impact on our patients' lives.
- Be a Licensed Professional Nurse or a Licensed Vocational Nurse licensed in the state in which you practice.
- Have at least 1 year of home health experience.
- Prior packet review/QI experience is preferred, as is coding certification.
- Must be currently licensed in the State of employment if applicable.
- Must possess excellent communication skills, the ability to interact well with a diverse group of individuals, strong organizational skills, and the ability to manage and prioritize multiple assignments.
