Role Overview

The primary responsibility of the Client Care Coordinator is to facilitate and oversee care services in collaboration with referral sources, clients, and staff, ensuring the provision of high-quality services. This role involves direct supervision of staff, support for clients, and maintaining effective communication.

Key responsibilities include:

  • Managing client and caregiver inquiries, addressing issues and complaints promptly and professionally.
  • Escalating complex concerns to the appropriate leadership team.
  • Utilizing various databases and systems to maintain accurate information regarding clients and caregivers.
  • Staying informed about legal and regulatory standards, ensuring all necessary documentation is current.
  • Conducting regular follow-up calls to assess client satisfaction and health status.
  • Coordinating caregiver schedules to ensure service continuity and accuracy in hours worked.
  • Maintaining confidentiality of sensitive information.
  • Participating in after-hours on-call rotation as part of quality assurance practices.
  • Performing general office duties to support operations.

Qualifications

The ideal candidate will possess a bachelor’s degree and relevant office experience, particularly in managing databases and systems. Proficiency in Microsoft Office applications is essential, along with strong attention to detail and the ability to work independently.

Successful candidates will demonstrate excellent communication and customer service skills, contributing positively to team dynamics. The ability to manage stress and conflict effectively is crucial.
