ROLE OVERVIEW:
- We are in search of a dedicated Engagement Care Coordinator to enhance our service delivery.
- The Engagement Care Coordinator will oversee the integration of both internal and external outreach initiatives for individuals receiving care.
- The preferred candidate will possess exceptional communication abilities, a comprehensive understanding of healthcare frameworks, and the capacity to establish strong connections with clients.
- Facilitate collaboration and coordination among team members.
- Perform evaluations to ascertain the needs and obstacles faced by individuals receiving care.
- Inform individuals about available resources.
- Champion the needs of individuals to guarantee they obtain suitable care and assistance.
- Track progress and modify plans as necessary.
- Record all interactions and interventions in compliance with regulatory standards and organizational protocols.
- Engage with community resources and organizations to strengthen support networks for individuals.
- Contribute to quality enhancement initiatives aimed at improving care delivery effectiveness.
- Manage multiple electronic health records and databases while maintaining data tracking through spreadsheets to oversee individual care.
- Master's Degree in Human/Social Services with a minimum of one year of experience in behavioral health services.
- Bachelor's Degree in Human/Social Services with three years of experience in specialized behavioral health services.
- Pre-Employment Drug Screening.
- Act33/34/73 clearances.
- Ability to gather and analyze information while exercising independent judgment and discretion in decision-making.
Work Schedule: This is an Exempt (Salaried) role. Primarily Monday through Friday, daytime, with potential flexibility for evenings based on client needs.
