Role Summary – Residential Services, Direct Care

Position: Registered Nurse / Licensed Practical Nurse

Reports to: Senior Regional Program Director

Key Responsibilities:

The selected candidate will provide nursing services tailored to client needs. Responsibilities include:

  1. Delivering routine nursing care.
  2. Administering medications and training staff as directed by a physician, ensuring accurate dosage and documentation.
  3. Coordinating medical appointments in compliance with OPD regulations.
  4. Contributing to the development of nursing components in Individual Support Plans (ISPs) for assigned clients.
  5. Providing monthly updates to the CEO or designated personnel regarding care plans and client needs.
  6. Training clients and staff on health and hygiene practices.
  7. Conducting nursing assessments and monitoring vital signs as necessary.
  8. Participating in admission and discharge processes based on medical evaluations.
  9. Reviewing hospital discharge summaries and providing necessary training to staff.
  10. Maintaining precise medical records in Therap medical health records.
  11. Documenting medical concerns and notifying relevant parties as needed.
  12. Assisting in infection control measures and training staff on health protocols.
  13. Developing internal medical guidelines in collaboration with healthcare professionals.
  14. Ensuring adherence to the nursing care plan for active treatment.
  15. Upholding client rights at all times.
  16. Participating in the orientation and training of new nursing staff.
  17. Complying with facility safety protocols and administrative policies.
  18. Completing medication records and coordinating with healthcare providers.
  19. Engaging in clinical treatment team meetings as scheduled.
  20. Renewing and reviewing physician orders for accuracy.
  21. Providing one-on-one nursing supervision when required.
  22. Assessing and addressing health issues as they arise.
  23. Interpreting lab results and reports.
  24. Supporting on-call staff with health-related inquiries.
  25. Monitoring medication errors and taking appropriate actions.
  26. Performing additional duties as assigned.
Qualifications:
  1. Graduation from an accredited nursing program with three years of experience in a healthcare setting, including at least two years as an RN/LPN.
  2. Current RN/LPN licensure.
  3. Knowledge of nursing practices related to individuals with intellectual and developmental disabilities.
  4. Familiarity with basic medical procedures and treatment plan development.
  5. Proficient in maintaining accurate medical documentation.
  6. Ability to foster positive relationships with clients and staff.
  7. Capability to work independently with minimal supervision.
  8. Experience in supervising lower-level staff.
  9. Current CPR/First Aid certification.
  10. Physical ability to perform job duties, including lifting and carrying up to 70 lbs.
  11. Completion of required training and orientation for continued employment.
  12. Preferred experience working with individuals with intellectual and developmental disabilities.
  13. Valid driver's license with an acceptable driving record.
  14. Must be at least 21 years of age.
  15. Clearance of background checks as required.
