Home Health LPN Full Time Position

Job Summary:
Aveanna Healthcare is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a full-time Home Health LPN. As an LPN, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to deliver exceptional patient outcomes.
Key Responsibilities:
- Deliver patient-centered care, adhering to physician orders and developed care plans.
- Conduct thorough assessments, develop care plans, and implement interventions to prevent or reduce risk.
- Provide education and support to patients and their families, promoting health literacy and self-management.
- Collaborate with the healthcare team to ensure seamless care transitions and optimal patient outcomes.
- Document patient care and progress, maintaining accurate and timely records.
Requirements:
- Active LPN license in the state of application.
- Valid CPR certification.
Preferred Qualifications:
- Medicare Skilled Nursing experience.
- Basic understanding of Oasis.
- 1-year LPN experience in a home health or healthcare setting.
