Job Summary

The PRN LPN Home Health will provide high-quality, patient-centered care to individuals in the comfort of their own homes. You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans that address the unique needs and goals of each patient.

Responsibilities

  • Administer skilled nursing care to patients in their homes, following physician orders and the Plan of Care.
  • Conduct ongoing assessments to identify the physical, psychosocial, and environmental needs of patients and their families.
  • Develop and implement individualized care plans that address the unique needs and goals of each patient.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Provide education and support to patients and their families to promote health, wellness, and independence.

Requirements

  • Current LPN licensure in the state of Florida.
  • Basic Cardiac Life Support (BCLS) certification.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to provide care in a home setting.
