Job Summary:

We are looking for an experienced LPN to deliver skilled nursing care to patients in their homes. The ideal candidate will have a strong background in home health care, excellent communication skills, and a passion for providing exceptional patient care.

Responsibilities:
  • Deliver quality patient care in compliance with physician orders under the direction of a Registered Nurse teammate.
  • Perform the continuum of hands-on client care through documentation and timely communication with the client's physician and other caregivers.
  • Recognize changes in patient needs and responses requiring intervention and implement care to prevent risk or reduce risk.
  • Educate clients and their family members based on client-specific needs.
Requirements:
  • An active LPN license in the state of application.
  • Valid CPR certification.
Preferred Qualifications:
  • Medicare skilled nursing experience.
  • Basic understanding of Oasis.
  • 1-year LPN experience in a home health or healthcare setting.
