Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Health team at HCA Florida Healthcare at Home. As a Home Health LPN, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

Key Responsibilities
  • Administer quality, skilled nursing care to patients in their homes, working in accordance with physician orders and the Plan of Care under the direction and supervision of a Registered Nurse and/or Director, Professional Services.
  • Perform ongoing assessments during each visit and document data in patient medical records, communicating significant findings, problems, or changes in the patient's condition to the supervising RN and/or Director of Professional Services and the physician.
  • Assess patient/caregiver's knowledge of disease process, treatment regimen, and/or medication administration, providing basic and specialized teaching as ordered in the Plan of Care and based on the patient's needs.
  • Identify and recommend other disciplines needed for quality care of the patient, collaborating with the healthcare team to ensure the provision of quality patient care.
Requirements
  • Basic Cardiac Life Support certification must be obtained within 30 days of employment start date.
  • Licensed Practical/Vocational Nurse (LPN/LVN) licensure.
  • Valid driver's license.
  • Vocational school graduate.
