Key Responsibilities
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare providers.
  • Provide skilled nursing interventions, including wound care, medication management, and education on disease management and prevention.
  • Conduct thorough assessments, identify patient needs, and prioritize interventions to achieve optimal outcomes.
  • Collaborate with interdisciplinary team members to ensure seamless care coordination and communication.
  • Document patient progress, outcomes, and any changes in condition, ensuring accurate and timely reporting.
Requirements
  • Current licensure as a Licensed Practical Nurse (LPN) in the state of Florida.
  • One year of clinical experience, with a preference for home health or gerontology experience.
  • Successful completion of a 30-hour IV Therapy Course.
  • Ability to maintain clinical competency and adapt to changing patient needs.
  • Valid driver's license and reliable transportation.
