About the Role

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Health team at HCA Florida Healthcare at Home. As a PRN LPN Home Health, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

Key Responsibilities
  • Administer skilled nursing care to patients in their homes, under the direction of a Registered Nurse and/or Director of Professional Services.
  • Conduct ongoing assessments to identify the physical, psychosocial, and environmental needs of patients, and document data in patient medical records.
  • Communicate significant findings, problems, or changes in the patient's condition to the supervising RN and/or Director of Professional Services and the physician, and document all findings, communications, and appropriate interventions.
  • Provide basic and specialized teaching as ordered in the Plan of Care and based on the patient's needs, and identify and recommend other disciplines needed for quality care of the patient.
Requirements
  • Licensed Practical/Vocational Nurse (LPN/LVN) licensure in the state of Florida.
  • Basic Cardiac Life Support certification within 30 days of employment start date.
  • Valid driver's license.
