Job Overview

Davis Behavioral Health is in search of a dedicated Case Manager to become a vital part of our Children, Youth, and Families Mental Health outpatient team. This role involves working closely with children and their families, providing essential case management, behavioral guidance, and skills enhancement services. The Case Manager plays a crucial role in helping clients navigate and utilize community resources effectively.

Key Responsibilities

The Case Manager will be responsible for:

  • Conducting comprehensive in-home assessments to identify needs and strengths.
  • Creating personalized service plans with clearly defined goals and objectives, updating them as necessary.
  • Connecting families with appropriate care systems to enhance their access to resources and services.
  • Advocating for clients facing challenges in accessing necessary care systems.
  • Delivering individual, family, and group skills training using evidence-based methods to alleviate symptoms and improve overall functioning, in collaboration with mental health professionals.
  • Supporting families in building natural support networks to foster independence from agency services.
  • Maintaining accurate clinical documentation in accordance with agency standards.
  • Coordinating care with other staff and community organizations as needed.
  • Participating in regular staff meetings and supervision sessions.
  • Meeting agency productivity expectations.
  • Performing additional duties as assigned.
Compensation

The compensation for this position is structured as follows:

  • $25.50 per hour for candidates licensed as a Social Service Worker (SSW) in the State of Utah, with a 1% increase for each year of licensure.
  • $21.50 per hour for candidates holding a bachelor's degree in a social science field without an SSW license, with a 1% increase for each year of licensure up to 10 years.
  • $18.25 per hour for candidates without a bachelor's degree, with a similar 1% increase for each year of licensure up to 10 years.
Work Schedule

This is a full-time position typically scheduled for weekdays, with the possibility of three evenings per week. Flexibility for overtime, weekend work, and evening or night shifts may be required based on operational needs.

Experience Requirements

Preference will be given to candidates who are licensed as a Social Service Worker (SSW). Those actively pursuing licensure or eligible for Targeted Case Management Certification may also be considered.

  • Flexibility in work hours, including evenings.
  • Proficiency in electronic medical records and computer skills.

Meeting monthly service expectations and productivity standards is required. Candidates must pass a pre-employment criminal background check and drug screening.
