Overview
Patient Resource Navigator: A Supportive Role
At Partners In Care, we pride ourselves on delivering compassionate patient-centered care. As a Patient Resource Navigator, you will play a vital role in supporting patients and their families by providing non-clinical case management and connecting them with community resources.

About the Role
This full-time position is based in Bend, Oregon, offering a competitive salary range of $22.61-$36.13 per hour (DOE). You will work Monday-Friday, 40 hours a week, enjoying generous paid time off and benefits.

Key Responsibilities
As a Patient Resource Navigator, your primary responsibilities include:
• Establishing and maintaining positive working relationships with current and potential patients and referral sources
• Acting as a liaison between the patient, family/caregivers, clinical team, and providers in coordinating care as guided and directed by the interdisciplinary team
• Serving as a resource for patients and/or their families to educate them about various services and programs
• Accurately documenting all care and information in the electronic medical record
• Effectively collaborating with all interdisciplinary team members, including the family and support systems
• Advocating for patients, interacting positively with various community and healthcare representatives
• Coordinating an array of services designed to support patients, including but not limited to caregiving, housing, transportation, and meals
• Addressing barriers to care, coordinate with outside resources and collaborates with the interdisciplinary team to meet the goals of care for the patient
• Providing services to patients and families, both face-to-face and telephone, based on needs and changes in the patients health status
• Preparing for, attends, and facilitates bi-weekly Hospice Interdisciplinary Team (IDT) meetings
• Supports the bereavement department with administrative tasks

Requirements
To succeed in this role, you should possess:
• A high school diploma or equivalent
• A Bachelor's degree or equivalent experience
• One year of home health/hospice care experience is preferred
• Excellent observation, verbal, and written communication skills
• Case management experience preferred
• Knowledge and understanding of support resources in Central Oregon are preferred
• Experience/understanding of psychosocial issues related to illness and end-of-life care
• Ability to work independently and collaboratively within an interdisciplinary team
• Compassion, empathy, and dedication to patient care
• Ability to pass a background test, drug screen, and TB test
• Current automobile insurance, a valid Oregon drivers license, and reliable transportation are required for those working in the field
