About the Role

We are seeking a dedicated and compassionate Registered Nurse (RN) to join our team as a Senior Hospice Nurse Coordinator at Heart 'n Home Hospice and Palliative.

Responsibilities
  • Making initial nursing evaluations within 48 hours of referral to determine eligibility for hospice services.
  • Identifying patients' and families' physical, psychosocial, and environmental needs, re-assessing as needed every 15 days.
  • Initiating and coordinating plans of care while maintaining accurate clinical notes regarding patients' conditions.
  • Documenting problems, goals, interventions, and patient/family response to hospice care.
  • Collaborating with patients/families, attending physicians, and other members of the Interdisciplinary Group (IDG) in providing daily patient and family care.

The RN Case Manager will assume full nursing responsibility for delivering the Plan of Care for each hospice patient.

Requirements
  • Experience: One year of clinical experience is required.
  • License Requirements: Current RN licensure in the state of practice and one year of clinical experience.
  • Certifications: Current CPR Certification is required.
  • Driver's License: A current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation are necessary.
