Job Description
The Patient Resource Navigator role is a vital part of our team, requiring exceptional communication and coordination skills. As a Patient Resource Navigator, you will support patients, families, and our social worker team by providing non-clinical case management and connecting them with community resources. Your responsibilities will include establishing and maintaining positive working relationships, acting as a liaison, serving as a resource, accurately documenting care, and effectively collaborating with the interdisciplinary team. You will also advocate for patients, interact with community representatives, and coordinate services designed to support patients.

Requirements and Qualifications
To be successful in this role, you should possess a high school diploma or equivalent, a Bachelor's degree or equivalent experience, and one year of home health/hospice care experience is preferred. You should also have excellent observation, verbal, and written communication skills, case management experience, and knowledge and understanding of support resources in Central Oregon. Additionally, you should be able to work independently and collaboratively within an interdisciplinary team, demonstrate compassion, empathy, and dedication to patient care, and pass a background test, drug screen, and TB test. Current automobile insurance, a valid Oregon drivers license, and reliable transportation are required for those working in the field.

Benefits and Compensation
We offer a competitive salary range of $22.61-$36.13 per hour (DOE), paid bi-weekly. Our benefits package includes two medical, dental, and vision plans; insurance starts on the 1st of the month following the hire date. You will also receive generous paid time off, a $25,000 life insurance policy, short and long-term disability, and a 403b retirement savings plan with employer match.
