Palliative Care Specialist

We are seeking a dedicated and compassionate Palliative Care Specialist to join our team at Partners In Care. As a Palliative Care Specialist, you will work closely with patients and their families to provide emotional support, counseling, and complex care management services.
Responsibilities- Conduct thorough psychosocial assessments of patients and families to identify support systems, coping mechanisms, and barriers to care.
- Utilize therapeutic techniques to facilitate patient-centered decision-making around health needs.
- Provide emotional support counseling and care coordination services to palliative care patients and families.
- Maintain open communication with healthcare providers to ensure seamless care transitions.
- Develop and implement personalized care plans that address the unique needs of each patient.
- Masters degree in Social Work from an accredited program.
- 1-3 years of experience in end-of-life care or a related field.
- Valid State of Oregon Clinical Social Worker license (LCSW).
- Specialized certification in palliative care social work is preferred.
- Excellent communication and interpersonal skills.
- Able to work independently and collaboratively within an interdisciplinary team.
- $43,000 - $63,000 per year, depending on experience.
