As a compassionate Care Coordinator at Partners In Care, you will play a vital role in delivering exceptional patient-centered care to individuals and families across Central Oregon.

Job Summary

We are seeking an experienced Hospice Social Services Specialist to join our interdisciplinary team. As a key member of our team, you will work closely with patients, families, and healthcare professionals to provide comprehensive social support services.

The ideal candidate will possess 1-2 years of experience in hospice/home health care or hospital settings, with a strong understanding of psychosocial issues related to illness and end-of-life care.

Responsibilities will include:

  • Conducting comprehensive psychosocial assessments on patients/families referred for Home Health and Hospice services.
  • Developing and implementing individualized plans of care to address identified needs and goals.
  • Providing a range of professional social work services, including counseling, emotional support, home care planning, patient/family education, resource identification, information and referral, advocacy, and other supportive services.
  • Collaborating with Bereavement services to complete risk assessments for identified primary bereaved both before and after a patient's death as indicated.
  • Maintaining collaborative relationships with community agencies/resources to support patient/family care and identify psychosocial needs.
