Florida, United States Imagine Pediatrics Full time
About Us

At Imagine Pediatrics, we are a dedicated and empathetic, multi-disciplinary medical group led by pediatricians, striving to create a world where every child with intricate medical needs receives the care and support they rightfully deserve. Driven by our mission to transform pediatric healthcare, we are at the forefront of virtual-first care services, which encompass care coordination and 24/7 access to specialized clinicians from the comfort of home. We do not replace a child's existing healthcare team; rather, we collaborate to provide ongoing support, enabling children to spend less time in hospitals and more time thriving at home and within their communities.

Each day, we courageously challenge outdated practices and seek innovative solutions to enhance the health and lives of children facing medical complexities. Together with caregivers, healthcare providers, and insurance plans, we dismantle the obstacles that hinder families from accessing quality care for their children. While we recognize that we cannot resolve every challenge in these children's lives, we are committed to tirelessly providing them, their families, and caregivers with the assistance and relief they require, precisely when they need it. We believe that a better approach to pediatric healthcare is attainable, and we are dedicated to realizing it today.

Our Core Values

We are guided by five fundamental principles:
  • Children first: We prioritize the best interests of children above all else, unwavering in our commitment to enhancing the lives of children and families by delivering exceptional care.
  • Build trust: We value the trusting relationships we cultivate with the children and families we serve, our partners, and one another. We strive to understand first and communicate transparently.
  • Innovate now: We reject the notion of 'this is how it's always been done.' Children and families are awaiting our assistance, and their courage compels us to continuously challenge assumptions and develop new methodologies.
  • Embrace humanity: We approach our work with empathy and authenticity, assuming competence and good intentions. When we falter, we view it as an opportunity to learn and grow stronger.
  • Unified team, diverse perspectives: We collaborate with families as a cohesive unit toward a common goal, championing diversity and recognizing the contributions of all.

Your Role

As an Outreach Team RN at Imagine Pediatrics, you will foster positive, long-lasting relationships with pediatric offices, specialty practices, hospital discharge planners, social workers, home health agency nurses, and other community team members to enhance the engagement of eligible patients and their families. You will collaborate with other members of the interdisciplinary team, including physicians, advanced practice providers, social workers, and navigators, as needed to enroll patients and caregivers.
  • Develop innovative strategies to enroll patients and families into our services.
  • Collaborate with the Director of Network & Engagement to establish connections with pediatricians, pediatric specialists, local hospitals, home health agencies, group homes, and clinics to drive patient and family engagement.
  • Manage relationships: Establish a regular meeting schedule with community partners to discuss our services, currently engaged patients and families, and eligible patients.
  • Build working relationships with practice managers, primary care providers, discharge planners, and key provider stakeholders to create formal business partnerships.
  • Comfortable conducting telephonic outreach to eligible families to enroll them in our program.
  • Utilize reporting and dashboards to prioritize telephonic and field-based outreach to members to enhance engagement.
  • Meet or exceed established monthly engagement targets.
  • Assist caregivers in the onboarding process of our integrated technology platform.
